Location: Kentucky Department of Public Health (hybrid)
The CDC Foundation is seeking a Rural Health Transformation Community Health Worker (CHW) Support and Partnership Coordinator to support the Kentucky Rural Health Transformation Program (RHTP). The Kentucky RHT is a multi-year, statewide initiative to modernize rural health systems and improve access, quality and sustainability for rural Kentuckians. The program operates across five interdependent initiatives — Rural Community Hubs, PoWER (perinatal health), Rapid Response to Recovery (behavioral health and crisis care), Rooted In Health (integrated/oral health), and Crisis To Care (community trauma care and EMS) — coordinated by a central program office.
This role provides statewide technical assistance and implementation support to organizations participating in the Rural Health Transformation Program (RHTP). This position partners with pilot sites, healthcare organizations, public health agencies and community partners to support the successful integration of Community Health Workers into clinical and community settings through evidence-based implementation strategies, workflow development, quality improvement and workforce development.
The Coordinator serves as a subject matter expert on Kentucky's CHW model, providing coaching, technical assistance, and program support while fostering collaboration, promoting adherence to state standards, and advancing sustainable CHW programs that improve health outcomes across Kentucky's rural communities. This role will be embedded within the Kentucky Department for Public Health's Kentucky Office of Community Health Workers, expanding the office's implementation capacity while serving as a key liaison between the Kentucky Office of Community Health Workers and RHTP leadership to advance statewide RHTP Community Health Worker initiatives.
