Job summary

Main Street Health, the nation's largest provider of value-based care exclusively serving rural America, is seeking a Health Navigator to ease the transition for patients and clinics. You will build trusted relationships, support administrative tasks, and coordinate care inside partner clinics alongside clinic staff.

You will meet with patients, enter data into our software, call to schedule visits, aid with referrals and navigate insurance and resources, all while maintaining compassionate

Qualifications

• GED or High School Diploma.
• Ability to work independently and in a fast-paced clinic setting.
• Strong communication and relationship-building skills.

Responsibilities

• Develop strong relationships with patients to assist them with their care
• Meet with patients during check-in, entering data into the Main Street software platform for reporting and tracking purposes
• Call patients to schedule visits or remind them to pick up medications
• Order and schedule various procedures, tests and screenings
• Identify and track progress on gaps in patient care
• Maintain records of patient interactions and communicate with providers using electronic health records
• Educate patients on healthcare options and insurance benefits
• Help patients access community resources
• Coordinate follow-up care after discharge
• Demonstrate compassion, adaptability, attention to detail, and teamwork
