Job summary

ACCESS Family Care is seeking a Rural Health Transformation Hub Navigator to connect rural residents with healthcare, behavioral health services, and community resources. The role focuses on advocacy, navigation, outreach, and coordinated care.

You will work with hospitals, clinics, EMS, public health agencies, and community partners to identify needs, coordinate referrals, and overcome barriers such as transportation, insurance, and access to care.

Qualifications

High School Diploma or equivalent.
Experience in community health work, care coordination, case management, patient navigation, Medicaid care management, behavioral health support, public health outreach or related fields.
Experience working with rural or underserved populations.
Proficiency using electronic systems for documentation and reporting.

Responsibilities

Conduct standardized screenings and intake assessments to identify healthcare, behavioral health, and social service needs.
Document screenings, referrals, outcomes, and follow-up in approved systems.
Coordinate and manage closed-loop referrals to ensure access to services.
Engage residents through outreach, field visits, and partner-site activities.
Maintain a caseload and follow up until services are completed.
