Job summary

Ozarks Community Hospital, Inc. in Sparta, MO seeks a Rural Health Transformation Program (RHTP) Navigator to serve as a frontline, community-based member of a Local Community Hub. The Navigator coordinates care across hospitals, FQHCs, RHCs, and community partners to connect residents with clinical, behavioral, and social services.

Frequent travel within Hub counties is expected. The role reports to the Hub Program Manager and focuses on structured screenings, referrals, and addressing barriers

Qualifications

• High school diploma or equivalent with relevant experience in community health work, care coordination, case management, or patient navigation.
• Experience working with rural communities and high-need populations facing barriers.
• Experience conducting intake and needs screening with referrals to completion.
• Ability to manage multiple active cases and document activities using digital systems.

Responsibilities

• Conduct standardized screening to identify clinical, behavioral, and social needs; document findings and referrals.
• Manage closed-loop referrals and warm handoffs across Hub partners; confirm service receipt and outcomes.
• Engage Medicaid members and other high-need residents through calls, visits, and outreach; provide health education and navigation.
• Help residents access Hub services including primary care, behavioral health, EPSDT, chronic disease management, and telehealth.
• Identify and resolve barriers to care; escalate complex issues to Hub Program Manager when needed.
• Maintain assigned caseload and follow-up until services are completed or transitioned.
