Job summary

The role requires travel within hub counties, a Monday–Friday schedule, and collaboration with hospitals, clinics, and public-health partners to improve access and outcomes for rural residents.

Qualifications

• High school diploma or equivalent and relevant community health experience.
• Experience with care coordination, referrals, and navigation.
• Strong communication and the ability to work across partners.

Responsibilities

• Screen residents for clinical, behavioral, and social needs and document findings.
• Manage closed-loop referrals across hospitals, clinics, and community partners.
• Engage high-need residents by calls, visits, and partner-site follow-up.
• Help residents access primary care, behavioral health, EPSDT, and transportation.
• Identify and resolve barriers to care; escalate when needed.
• Maintain an assigned caseload and document outcomes and barriers.
• Participate in hub huddles and share frontline insights.
