Fourrivers is seeking a Rural Health Transformation Program Navigator to join the community-based Hub in Missouri. The role focuses on screening, intake, referrals, and ongoing care coordination across hospitals, FQHCs, RHCs, and community partners.

As a frontline team member, you will engage Medicaid members and high-need residents, document outcomes, and support transitions between providers, all while contributing to Hub operations and quality improvement efforts.

Qualificações

• High school diploma or equivalent is required.
• Experience in community health work or care coordination is preferred.
• Strong verbal and written communication skills are essential.
• Ability to manage multiple cases and follow up until completion.
• Experience documenting activities using digital systems (CIE, EHR) is desirable.

Responsabilidades

• Conduct screening and intake to identify clinical, behavioral, and social needs and document findings in CIE.
• Manage closed-loop referrals across Hub partners and ensure service receipt and outcomes.
• Engage high-need residents through calls, visits, and outreach; provide health education and navigation.
• Help residents access Hub-supported services including primary care, behavioral health, EPSDT, and transportation.
• Identify and resolve barriers to care; escalate complex issues per Hub protocols.
• Maintain assigned caseload with structured follow-up until services are completed or transitioned.
• Contribute to Hub meetings and improve referral pathways and workflows.
• Track outreach and referral data for Hub, RCN, and RHTO; share insights.
• Escalate urgent concerns to licensed staff as needed.
• Support organizational initiatives and community outreach to promote access to care.
