Job summary

Qualifications

• Eligible for registration as a nurse in the UK.
• Experience in community or home-based care is desirable.
• Willingness to work across a rural locality and collaborate with multi-disciplinary teams.

Responsibilities

• Provide high-quality, person-centred nursing care in patients' homes and community settings.
• Carry out assessments and care planning, wound management and long-term condition support.
• Deliver palliative and end-of-life care and work with other health and social care professionals.
