Job summary

NHS Ayrshire & Arran is seeking a Community Mental Health Nurse with Older Adult Mental Health experience for Caithness. The role involves assessment, care planning, implementation and evaluation in partnership with clients, families and carers. You will monitor risk, liaise with psychiatrists and primary care, and work within a multidisciplinary team.

Responsibilities

• Assess adults 65 years and older or memory concerns as appropriate.
• Develop, implement and evaluate care plans with clients, families and carers.
• Monitor ongoing risk and liaise with psychiatrists, GPs and social care services.
• Coordinate with MDT and supervise students and support workers occasionally.

Job description

We are looking for an enthusiastic Community Mental Health Nurse with Older Adult Mental Health experience in Caithness. Your role will be with adults over the age of 65 or of any age if a memory concern. It will involve assessment, care planning, implementation and evaluation of the care plan, in partnership with the client and family and carers. You will also be involved in assessment and management of ongoing risk. The role will involve medication monitoring, liaising with Psychiatrists, Primary Care and with other Social Care services, and referring on to other services as appropriate. You will be an active member of an MDT. You will be supervising students and Support Workers on occasion.

You are required to be registered with the NMC and work within code of conduct at all times.

The applicant would be required to travel efficiently and effectively to undertake the role. A driving licence is essential.
