Job summary

Heartland Family Service seeks a Care Team Specialist to support rural clients across Southwest Iowa. You will coordinate care, track assessments (PHQ-9, DLA-20, C-SSRS), and ensure timely data entry and reporting within the CCBHC framework.

The role emphasizes collaboration with Zion staff, data sharing, and continuity of care, with a 37.5-hour week and an hourly wage of $19.74–$23.54, depending on experience.

Qualifications

• Bachelor’s degree in psychology, social services, sociology or related field is preferred.
• One year experience in health and human services or related field is desired.

Responsibilities

• Assist with releases of information, consents for treatment, and required agency documentation.
• Track required assessments and due dates to ensure compliance with CCBHC and agency requirements (PHQ-9, DLA-20, Columbia Suicide Severity Rating Scale).
• Ensure accurate data collection, entry, and record maintenance.
• Monitor timeliness and accuracy of assessments and address barriers to completion.
• Glean information and document assessments at admission and ongoing intervals; identify client strengths and goals.
• Advocate for client needs and promote engagement in services.
• Connect clients to physical health, behavioral health, housing, education, employment, and social services.
• Coordinate with client support networks and other providers to support care.
