Job summary

Heartland Family Service seeks a Care Team Specialist to improve access to care in rural Southwest Iowa, coordinating services and strengthening collaboration with Zion to support a data-driven, client-centered care model.

The role tracks assessments (PHQ-9, DLA-20, C-SSRS, etc.), ensures timely documentation, and maintains data sharing for the Certified Community Behavioral Health Clinic framework, while acting as a liaison across teams.

Qualifications

• Bachelor’s degree in psychology, social services, sociology or related field.
• One year experience in health and human services or related field.

Responsibilities

• May assist with completing releases of information, consents for treatment, and all required agency documentation.
• Track required assessments, including due dates, completion status, and follow up needs to ensure compliance with CCBHC and agency requirements.
• Insure accurate data collection, entry, and record maintenance.
• Monitor timeliness and accuracy of assessment completion and address barriers to completion with the care team.
• Coordinate with the client’s support network, including family members, healthcare providers, social service agencies, and legal representatives.
