Job summary

Sweetwater Care is seeking an LVN to provide nursing care within scope of practice in our facility. You will implement physician orders, monitor patient needs, and educate families while adhering to standards and center policies.

The role emphasizes timely medication and treatment administration, effective communication with the care team, and participation in rounds with physicians as required. Related experience in SNF/LTC is preferred.

Qualifications

• Licensed LVN in CA (per qualification).
• 1 year nursing management experience preferred.
• Experience in SNF/LTC settings preferred.

Responsibilities

• Provide nursing care per standards and policies.
• Administer medications timely according to facility policy.
• Administer treatments as ordered and per policy.
• Receive, transcribe and carry out physician orders when permitted.
• Make rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
