Willow Valley Communities is looking for a compassionate and detail-oriented PRN Registered Nurse Assessment Coordinator (RNAC) at The Glen. This role focuses on ensuring clinical excellence and advising interdisciplinary teams in a supportive work environment.

Qualifications

• Must have an active Pennsylvania RN license.
• Knowledge of OBRA and Department of Health regulations is required.
• Prior RNAC experience or MDS/Resident Assessment Certification is preferred.

Responsibilities

• Ensure accurate and timely completion of the MDS & Care Planning Process.
• Act as a liaison between nursing and interdisciplinary team members.
• Evaluate Residents for significant changes and support Medicare eligibility.
• Provide guidance on MDS processes and best practices.
• Maintain documentation integrity and coordinate submissions.
• Participate in Quality Assurance initiatives and ongoing care improvements.
• Provide hands-on nursing support when needed.

What You’ll Do

• Take part in MDS & Care Planning ProcessEnsure accurate, timely completion and compliance with federal, state, and facility standards.
• Collaborating Across TeamsAct as the liaison between nursing and interdisciplinary team members to ensure seamless care coordination.
• Driving Clinical ExcellenceEvaluate Residents for significant changes, initiate assessments, and support Medicare eligibility determinations.
• Educating & Empowering StaffProvide ongoing education and guidance on MDS processes and care planning best practices.
• Ensuring Compliance & AccuracyMaintain documentation integrity, coordinate submissions, and track Resident status updates with regulatory agencies.
• Supporting Quality ImprovementParticipate in Quality Assurance initiatives, committee work, and ongoing care improvement efforts.
• Stepping in When It Matters MostProvide hands‑on nursing support when needed to ensure Residents receive the highest level of care.

What You Bring

• Active Pennsylvania Registered Nurse (RN) license
• Knowledge of OBRA and Department of Health regulations
• RNAC experience or MDS/Resident Assessment Certification (preferred)
• Strong attention to detail, collaboration, and communication skills

Schedule & Work Environment

• PRN / As‑needed flexibility
• Daytime hours
• 100% on‑site (non‑remote)
