Job summary

About the Role

As an RNAC Registered Nurse Assessment Coordinator, you will be responsible for conducting thorough assessments of residents to identify their medical, social, and psychological needs. You will also develop and implement individualized care plans, coordinate with healthcare providers, and ensure seamless transitions between care settings. Your exceptional communication skills will enable you to effectively collaborate with residents, families, and healthcare team members to achieve optimal outcomes.

Requirements
  • Current Registered Nurse in the State of Pennsylvania in good standing
  • 1+ years of recent experience in Minimum Data Set assessments and reporting.
  • Strong understanding of Medicare and Medicaid regulations, including RAI
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work effectively in a fast-paced environment with multiple priorities
  • Strong analytical and problem-solving skills
