Job summary

Meadow View Nursing Center in Berlin, PA seeks an experienced Registered Nurse Assessment Coordinator (RNAC) to lead the resident assessment and care planning process in compliance with federal, state, and local guidelines. The RNAC will manage MDS/RAI activities, schedule OBRA and Medicare assessments, coordinate care plans, and communicate coverage status with residents and families.

PA LTC experience preferred; RAC-CT a plus.

Qualifications

• Current RN license is required.
• Basic computer and EHR/PCC knowledge and experience is required.
• Preferred certification: RAC-CT.
• Previous experience with Resident Minimum Data Sets and Care planning preferred.
• Long term care experience is highly preferred.

Responsibilities

• Coordinate development and accurate completion of resident assessment and care plans in compliance with federal, state and local guidelines.
• Maintain strong knowledge of MDS, the RAI and care planning process; implement CAAs.
• Schedule OBRA and Medicare/managed care assessments and maintain entry/re-entry records.
• Inform residents and families about Medicare/managed care coverage status and document appropriately.
• Collaborate with CHC case managers for Managed Medicaid providers and participate in committees as directed.

We are currently seeking an experienced Registered Nurse Assessment Coordinator-(RNAC) to join our collaborative team to support regulatory compliance, resident centered care and survey readiness.

Excellent communication and interpersonal skills are required for this multifaceted position.

This role is responsibleto conduct and coordinate the development and accurate completion of the resident assessment and care plan process in compliance with all federal, state and local guidelines which govern the facility. Strong knowledge of MDS, the RAI and care planning process, including the implementation of CAAs.

Coordinates scheduling of OBRA and Medicare/managed care assessments, entry/re-entry records, and care plan reviews in accordance with the most recent regulatory guidelines.

Inform residents, responsible parties and staff of Medicare/managed care coverage status through appropriate documentation in accordance with all federal, state and facility guidelines. Provides updates as necessary. Works with Community Health Choice (CHC) case managers for Managed Medicaid providers Participate in committees as directed.

In-depth knowledge of Pennsylvania case mix, Medicare PDPM payment system and requirements for managed care coordination. Knowledge of Quality Measures.
