Job summary

WeCare Centers at The Green Home Care Center in Wellsboro, PA seeks a motivated RNAC to coordinate the Resident Assessment Instrument (RAI) process, including MDS, care planning, and quality assurance. The RNAC will ensure accurate assessments, develop individualized plans with an interdisciplinary team, and monitor outcomes.

The ideal candidate holds an RN license in PA, with MDS/LTC experience and RAC-CT certification preferred, and strong clinical assessment and regulatory knowledge.

Qualifications

• Current RN license in PA.
• MDS/LTC experience preferred.
• RAC-CT certification preferred.
• Strong clinical assessment skills.
• Knowledge of Medicare and Medicaid regulations.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team environment.

Responsibilities

• Coordinate the RAI process, including completion of the MDS and care planning
• Ensure accuracy and completeness of resident assessments
• Collaborate with interdisciplinary team members to develop individualized care plans
• Monitor and evaluate the effectiveness of care plans
• Ensure compliance with Medicare and Medicaid regulations
• Provide education and training to staff on the RAI process and care planning
• All other job duties given by administration
