Job summary

Heritage Pointe Rehabilitation and Nursing Center is seeking a Registered Nurse Assessment Coordinator (MDS/RNAC) to manage patient assessments and coordinate care.

You will collect information, provide regular reports to physicians, and work with our MDS system to ensure accurate care plans while upholding nursing standards and facility policies.

The role requires RN licensure in PA and prior RNAC or MDS experience, with a focus on clinical accuracy and compliant documentation.

Qualifications

• Experience in an RNAC or MDS position.
• Active Registered Nurse license in PA (PA RN license).

Responsibilities

• Assess patients and gather information for care planning.
• Provide regular reports to doctors and other medical professionals.
