Qualifications

• Current RN license in PA is required.
• MDS/LTC experience is required.
• Certification in RAC-CT preferred.
• Strong clinical assessment skills.
• Knowledge of Medicare and Medicaid regulations.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team environment.

Responsibilities

• Coordinate the RAI process, including completion of the MDS and care planning.
• Ensure accuracy and completeness of resident assessments.
• Collaborate with interdisciplinary team to develop individualized care plans.
• Monitor and evaluate effectiveness of care plans.
• Ensure compliance with Medicare and Medicaid regulations.
• Provide education and training to staff on RAI process and care planning.
• All other duties as assigned by administration.
