Qualifications
- Bachelor’s degree in Nursing from ACEN/CCNE accredited program.
- Minimum 24 months of full-time case management or care coordination recently.
- Current, active RN license and BLS certification.
- CCM or ANCC certification or eligible required.
Responsibilities
- Identify patients needing CM and coordinate care plans.
- Provide CM education and advocate for timely care delivery.
- Collaborate with PCM teams and document in EHR per guidelines.
- Perform BH CM services and report outcomes to teams.
- Maintain CM registries and participate in meetings/webinars.
