Qualifications

• Master’s degree in nursing and active RN license.
• Minimum five years of nursing experience.
• Minimum 3 years in clinical case management (ADN) or 2 years (BSN) full-time.
• CCM or ANCC certification (or eligible with required experience).
• BLS and ACLS.
• Excellent organizational and self-directed work habits.
• Strong verbal and written communication; proficient with Microsoft Office.

Responsabilités

• Coordinate case management for wounded warriors and manage care plans.
• Collaborate with medical teams and refer to appropriate specialists.
• Document patient progress and outcomes in the care system.
• Ensure compliance with regulatory and facility policies.
