ERP International, LLC is seeking a full-time Registered Nurse (RN) Case Manager to support the 1st Special Operations Medical Group at Hurlburt Field, FL.

The role emphasizes comprehensive case management across inpatient and outpatient settings, with opportunities for CME and a robust benefits package. Qualifications

• Associates Degree of Nursing required.
• Possess one of multiple recognized nursing certifications.
• One year RN Case Manager experience preferred.

Responsibilities

• Participate in all phases of the Case Management Program and ensure standards of care.
• Provide nursing expertise about the CM process: assessment, planning, implementation, coordination, monitoring.
• Develop and implement local CM strategies across inpatient, outpatient, onsite and telephonic settings.
• Develop tools to support case management and integrate CM with UM and social work case management.
• Maintain liaison with community agencies and document patient care data.
