Job summary

Nightingale Nursing & Rehabilitation Center in Erie, PA is seeking a dedicated RN Wound Care Nurse to join our clinical team. You will assess wounds, implement treatment plans, and collaborate with physicians to promote healing and quality of life for residents.

The role supports on-site care with documentation, education for residents and families, and participation in on-call rotation. PA RN licensure is required, with wound care experience preferred and WCC/WOCN certification a plus.

Qualifications

• Current RN license in Pennsylvania in good standing.
• Wound care experience in skilled nursing, rehab, LTC, or acute care preferred.
• Wound care certification (WCC, WOCN or similar) preferred but not required.
• Strong assessment, documentation, and clinical decision-making skills.
• Ability to work independently while collaborating with interdisciplinary teams.
• Excellent communication and organizational abilities.
• Commitment to compassionate, resident-centered care.

Responsibilities

• Conduct comprehensive wound assessments and ongoing evaluations of resident skin integrity.
• Develop, implement, and monitor individualized wound care treatment plans.
• Perform wound treatments and dressing changes per physician orders and protocols.
• Monitor and document wound progress, interventions, and outcomes accurately and timely.
• Collaborate with physicians, nursing staff, therapy, and dietary teams to optimize healing outcomes.
• Provide education to residents, families, and staff regarding wound prevention and treatment.
• Participate in quality assurance and infection prevention initiatives related to wound management.
• Ensure compliance with state, federal, and facility regulations regarding documentation and care delivery.
• Serve as a clinical resource for pressure injury prevention and skin care management.
• Participate in the facility's on-call rotation schedule as assigned.

Monday-Friday Position. This position will be part of the facilities on-call rotation

Nightingale Nursing & Rehabilitation Center is seeking a dedicated and detail-oriented RN Wound Care Nurse to join our clinical team. As a RN Wound Care Nurse, you will play a critical role in promoting healing, preventing complications, and improving quality outcomes for our residents through comprehensive wound assessment and management.

Responsibilities:

• Conduct comprehensive wound assessments and ongoing evaluations of resident skin integrity

• Wound Care Certification (WCC, WOCN, or similar) preferred, but not required
• Strong assessment, documentation, and clinical decision-making skills
• Ability to work independently while collaborating effectively with interdisciplinary teams
• Excellent communication and organizational abilities
