Job summary

Tufts Medicine Care at Home is seeking a Wound Care Program leader with Massachusetts RN licensure and WOCN certification to develop, implement, and oversee high-quality wound care services in Lowell, MA and surrounding areas.

The role requires strong leadership, clinical expertise across wound care, and collaboration with nurse managers, physicians, and community resources to ensure safe, effective patient care and ongoing program advancement.

Qualifications

• Massachusetts RN Licensure.
• Wound, Ostomy, Continence Nursing (WOCN) certification.
• Three (3) years of WOCN experience.
• Two (2) years of Home Health experience.

Responsibilities

• Provides leadership and guidance for the Wound Care Program and nursing staff.
• Assists in care delivery planning, case conferencing, and referrals to Ethics Committee.
• Ensures compliant care delivery and regulatory standards.
• Oversees wound care documentation accuracy and completeness.
• Manages supply orders and collaborates with vendors for formulary.
• Coordinates referrals and collaborates with care teams and discharge planning departments.
