Job summary

Royal Health Group seeks an experienced MDS Nurse for Hanover Terrace in Hanover, NH. You will coordinate MDS activities, ensure timely PPS/MDS assessments, and support interdisciplinary care planning to promote optimal resident outcomes.

Qualifications

• NH RN license required and current.
• Experience with MDS, PPS, and CMS processes.
• Strong communication with residents, families, and team.
• Familiarity with HIPAA regulations and state/federal laws.
• Proficient in PCC EMR and care planning.

Responsibilities

• Facilitates weekly skilled Medicare meetings and provides clear communication regarding skilled coverage criteria.
• Transmits MDS to CMS and follows up to validate any errors or messages revealed on final validation reports.
• Completes all resident Interviews and PPS MDS assessments within regulatory time frames including CAAs.
• Drives the development of the Interdisciplinary Care Plan and updates subsequent MDS Assessment.
• Participates and reviews invoices as indicated for skilled patients under consolidated billing.
• Maintains a current PPS schedule while setting all PPS MDS ARDs within time frames in the MDS software program.
• Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.
• Participate in case management activities to ensure continuity of care.
• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.
• Collaborate with healthcare team members to deliver comprehensive resident care in our facilities.
• All other duties as assigned.
