Job summary

Derry Rehabilitation and Nursing Center is seeking a skilled RN to join our on-site team in New Hampshire, focusing on comprehensive resident assessments, MDS 3.0 processing, and care planning. You will collaborate with staff to ensure timely, accurate documentation and high-quality resident care.

This role requires long-term care experience, strong organizational skills, and excellent communication to support a patient-centered environment and teamwork within the facility.

Qualifications

• Valid RN License.
• MDS 3.0 experience required (2+ years).
• Long term care experience required (2+ years).
• Must be highly organized, professional & motivated.
• Excellent communication skills.
• Should be friendly and a team worker.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining & updating all care plans and assessments as required.
• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident & Facility information at all times.
