Job summary

Seabrook Village by Erickson Senior Living seeks a hands-on Wellness Manager to lead a 48-bed Assisted Living neighborhood, ensuring safe, high-quality, person-centered care while mentoring nursing staff.

You will coordinate clinical services, supervise medication management, and partner with the Assisted Living Manager to maintain care plans and assessments, driving staff development and resident outcomes.

Qualifications

• Current RN license in the state of operation.
• Minimum of 3 years of experience as a registered nurse is required.
• Management experience within Home Health, Assisted Living, and/or Memory Care is preferred.
• Experience with an electronic medical record (EMR) is preferred.

Responsibilities

• Ensure compliance with state, local, and company policies and regulations.
• Provide clinical oversight and care to residents in Assisted Living and Memory Care, including medication, pain, wound care, nutrition and respiratory care.
• Collaborate with the Assisted Living Manager for resident pre-screening/admission, re-admission, and ongoing condition assessments.
• Maintain and update individualized service plans reflecting resident needs in collaboration with care team.
• Coordinate outside services (hospice, therapy, home health) and integrate plans into resident care plans.
• Track clinical quality indicators and implement corrective actions where needed.
• Participate actively in care and safety meetings and performance improvement activities.
