Seabrook Village by Erickson Senior Living in Tinton Falls, NJ is seeking a compassionate Wellness Manager to join our clinical leadership team. You will oversee an Assisted Living neighborhood, ensuring safe, high-quality, person-centered care and mentor care teams.

The role focuses on clinical oversight, regulatory compliance, and collaboration with ALM and memory care staff to develop individualized service plans and manage care across residents, while contributing to quality improvement and

Qualifications

• Current RN license in the state in which they operate.
• Minimum of 3 years of experience as a registered nurse.
• Management experience within a Home Health, Assisted Living, and/or Memory Care licensed setting preferred.
• Experience and comfort working with an electronic medical record (EMR) preferred.

Responsibilities

• Oversee an Assisted Living neighborhood ensuring safe, high-quality, person-centered care.
• Coordinate clinical oversight and care, including medication management and wound care.
• Partner with Assisted Living Manager on resident pre-screening/admission, re-admission, and ongoing assessments.
• Maintain and update individualized service plans in collaboration with ALM team.
• Coordinate outside services and ensure plans are integrated into resident plans.
• Track clinical quality indicators and develop corrective action plans.
• Participate in continuing care meetings and PI/RM/S committees.
