Position: PM Visit Nurse

As a PM Visit Nurse, you will ensure the delivery of quality home care services, coordinating care that aligns with our standards and philosophy. Responsibilities include:

1. Assess patient and family/caregiver conditions accurately during visits, including physiological, psychosocial, and spiritual aspects.
2. Identify and address educational needs of patients and families.
3. Provide patient care according to physician orders and agency policies.
4. Complete all required documentation accurately and timely.
5. Implement and evaluate new physician orders precisely.
6. Maintain accurate schedules, payroll, and logs.
7. Collaborate effectively with patients, families, care providers, and staff.
8. Communicate patient issues promptly to relevant healthcare professionals.

Minimum Qualifications:

• Degree from an accredited nursing school (BSN preferred)
• Current WI RN license
• 1-2 years of nursing experience preferred, with at least one year in acute, long-term, or home care
• Valid WI driver's license and reliable, insured vehicle

This position is for 2nd shift, Monday-Friday (2:30 pm - 11:00 pm), with no weekends but includes holiday rotation. Per visit compensation plus up to a $15,000 hiring bonus.
