Horizon Home Care and Hospice is seeking experienced nurses to join our top-notch Home Care Team.

As a PM Visit Nurse, you will be responsible for ensuring the delivery of quality home care services and coordinating services that reflect the philosophy and standards of home care nursing for assigned patients. You will provide services at quality levels consistent with professional standards of practice.

1. Assess patient and family/caregiver conditions accurately and thoroughly during each visit, including physiological, psychosocial, and spiritual aspects.
2. Identify and address patient and family educational needs.
3. Provide patient care according to physician orders and agency policies.
4. Complete all required patient and agency paperwork accurately, legibly, and promptly to reflect the care provided.
5. Transcribe, implement, and evaluate new physician orders accurately.
6. Ensure field staff schedules, payroll, and daily logs are accurate, legible, and timely.
7. Work cooperatively with patients, care providers, family members, and agency staff.
8. Communicate patient issues and concerns to the Case Manager, healthcare professionals, or Clinical Manager in a timely manner.

Minimum Requirements:

• Degree from an accredited School of Nursing (BSN preferred)
• Current WI RN license
• 1-2 years of related nursing experience preferred
• Minimum of one year RN experience in acute care, long-term care, or home care setting required
• Valid WI Driver's License and reliable, insured vehicle

Shift Details:

2nd shift, Monday-Friday (2:30 pm - 11:00 pm). No weekends, includes holiday rotation.

Per visit, exempt position with up to a $15,000 hiring bonus.
