Horizon Home Care and Hospice, a 9-time Top Workplace, is seeking experienced nurses to join our dedicated Home Care Team.

Position: PM Visit Nurse

As a PM Visit Nurse, you will ensure the delivery of high-quality home care services, coordinate care that aligns with our philosophy and standards, and provide services at levels consistent with professional practice.

1. Assess patient and family/caregiver conditions accurately during visits, considering physiological, psychosocial, and spiritual factors.
2. Identify and address educational needs of patients and families.
3. Administer patient care according to physician orders and agency policies.
4. Complete all required documentation accurately and promptly to reflect care provided.
5. Implement and evaluate physician orders meticulously.
6. Maintain accurate and timely schedules, payroll, and logs.
7. Collaborate effectively with patients, families, care providers, and staff.
8. Communicate patient issues and concerns promptly to relevant team members.

Minimum Requirements:

• Degree from an accredited nursing school (BSN preferred)
• Current WI RN license
• 1-2 years of nursing experience preferred, with at least one year in acute care, long-term care, or home care
• Valid WI driver's license and reliable, insured vehicle

Schedule: 2nd shift, Monday-Friday (2:30 pm - 11:00 pm). No weekends, includes holiday rotation.
