Position: PM Hospice Admission/Visit Nurse

As a Hospice Nurse, you will ensure the delivery of quality home hospice services and coordinate care that aligns with the philosophy and standards of hospice nursing. Your responsibilities include:

1. Assess patient and family/caregiver conditions accurately during each admission or visit, including physiological, psychosocial, and spiritual aspects.
2. Identify and address educational needs of patients and families.
3. Provide patient care according to physician orders and agency policies.
4. Complete all required documentation accurately, thoroughly, legibly, and promptly.
5. Transcribe, implement, and evaluate new physician orders.
6. Collaborate effectively with patients, families, care providers, and agency staff.
7. Communicate patient issues and concerns to the appropriate team members in a timely manner.

Qualifications:

• Degree from an accredited nursing school (BSN preferred)
• Current WI RN license
• 1-2 years of related nursing experience preferred
• Minimum of one year RN experience in acute care, long-term care, or home care
• Valid WI Driver's License, reliable and insured vehicle

Work Schedule:

Monday-Friday, 2nd Shift (2:00 pm - 10:30 pm or 12:00 pm - 8:30 pm or 1:00 pm - 9:30 pm), no weekends, includes holiday rotation.

Per visit, exempt position with up to a $15,000 hiring bonus. Focused on hospice home care services.
