Premier Rehab & Healthcare at Burlington is seeking an experienced Unit Manager to oversee our long-term care and rehabilitation nursing unit. You will lead daily operations, supervise staff, coordinate resident care, and ensure high-quality outcomes in collaboration with physicians and therapists.

The role requires an active RN license in Vermont, leadership experience in skilled nursing or rehab, strong clinical judgment, and a commitment to resident safety and dignity.

Qualifications

• Current RN license (VT) required.
• Leadership or supervisory experience preferred.
• Experience in skilled nursing/long-term care rehab preferred.
• Strong clinical assessment and critical-thinking skills.
• Knowledge of LTC regulations (federal/state).

Responsibilities

• Oversee daily clinical and operational activities of the long-term care unit.
• Lead and mentor nursing staff to ensure safe, high-quality resident care.
• Coordinate with physicians, therapists, and care team for resident needs.
• Develop and update individualized resident care plans with the team.
• Monitor assessments, treatments, documentation, and outcomes.
• Ensure care aligns with physician orders, policies, and standards.
• Coordinate admissions, discharges, transfers, and transitions of care.
• Ensure compliance with federal and Vermont LTC regulations.
• Provide leadership, coaching, and professional development guidance.
• Identify training needs and support ongoing development.
• Participate in quality improvement initiatives.
• Review performance and seek improvements in care delivery.
• Communicate effectively with residents, families, and team.
• Address concerns and ensure follow-up and resolution.
• Promote infection prevention, safety, dignity, and a positive environment.
• Foster teamwork, accountability, and collaborative culture.
