Premier Rehab & Healthcare at Burlington is seeking an experienced Unit Manager – Long-Term Care (LTS) to join our nursing leadership team. You will oversee the daily operations of the Long-Term Rehabilitation unit and ensure resident-centered care.

The role includes supervising nursing staff, coordinating interdisciplinary services, monitoring outcomes, and ensuring compliance with state and federal regulations, while fostering a collaborative team environment.

Qualifications

• Current Vermont RN license.
• Previous leadership or unit-management experience.
• Experience in rehabilitation, skilled nursing, subacute rehab, or long-term care.
• Strong clinical assessment and critical-thinking skills.
• Knowledge of state and federal LTC regulations.
• Excellent communication and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced healthcare environment.
• Commitment to resident-centered care.

Responsibilities

• Oversee daily operations of the Long-Term Rehabilitation unit.
• Supervise nursing staff to ensure high-quality resident care.
• Collaborate with physicians, therapists, social workers, and other interdisciplinary team members.
• Coordinate and implement individualized resident care plans.
• Monitor resident assessments, treatments, and clinical outcomes.
• Ensure care delivery aligns with facility protocols and safe nursing practices.
• Participate in admissions, discharges, and care transitions.
• Ensure compliance with state, federal, and facility regulations.
• Provide leadership, mentoring, and support to nursing staff.
• Participate in quality improvement initiatives and performance monitoring.
• Communicate effectively with residents, families, and care team.
• Foster a collaborative, resident-focused environment.
• Support staff development and promote high standards of clinical care.
