You will develop and update individualized care plans, complete admissions and discharges, triage acute changes, ensure regulatory compliance, and mentor staff.

Connecticut license required with long‑term care experience; salary $95,000–$104,000 per year, negotiable based on experience.

Qualifications

• Licensed to practice as an RN in Connecticut.
• Long-term care experience required.
• Strong leadership and team mentorship abilities.
• Ability to develop and update care plans.

Responsibilities

• Develop, implement, and update individual care plans.
• Complete admissions, discharges, and other assessments.
• Manage triage, interventions, and physician orders; collaborate with families.
• Ensure compliance with health and safety regulations.
• Audit care documentation for quality and compliance.
• Communicate resident changes to families and the interdisciplinary team.
• Provide ongoing training, orientation and scheduling oversight for staff.
