Job summary

WeCare Centers at Foltsbrook Care Center in Herkimer, NY, is seeking an experienced RN Unit Manager to lead a long-term care unit. This leadership role combines hands-on clinical oversight with administrative accountability to ensure resident-centered care and regulatory compliance.

The ideal candidate will have SNF or LTC leadership experience, strong staff mentoring skills, and a commitment to quality outcomes, survey readiness, and teamwork across disciplines.

Qualifications

• Active RN license in Pennsylvania (PA).
• Experience in SNF or Long-Term Care settings.
• Strong leadership and communication skills.
• Experience with dementia care and care planning is a plus.

Responsibilities

• Manage and coordinate all clinical care on an assigned unit.
• Lead, mentor, and support licensed and non-licensed nursing staff.
• Ensure compliance with state, federal, and CMS regulations.
• Participate in care planning, admissions, and clinical documentation.
• Support infection control, quality improvement, and resident safety initiatives.
• Collaborate with physicians and interdisciplinary teams to deliver resident-centered care.
