Job Description: Charge Nurse at The Avenue at North Ridgeville

The Avenue at North Ridgeville is a Post-Hospital Rehabilitation and Skilled Nursing Care Facility that offers a distinctive environment, including hotel-like accommodations and private suites. We are committed to providing excellent physical, medical, emotional, and spiritual support in a comfortable and respectful environment. Our facility offers some of the area's highest levels of rehabilitation, nursing, and medical support for individuals requiring post-hospital care. We seek a cheerful, friendly individual to administer care to our residents.

Qualifications:
  • Genuine interest in geriatric nursing.
  • Ability to make sound judgments and decisions.
  • Physically fit, mentally and emotionally stable.
  • Familiarity with the facility's policies and procedures.
  • Effective interpersonal communication skills.
Job Responsibilities:
  1. Understand that 100% occupancy is crucial to our success.
  2. Conduct rounds across all departments and coordinate with nursing staff.
  3. Report any issues encountered during rounds to the Administrative Office.
  4. Notify the Administrative Office of changes in residents' conditions.
  5. Collaborate with other department heads and consultants as needed.
  6. Supervise nursing personnel across all shifts.
  7. Assist with staff education, meetings, and orientation.
  8. Review nursing care plans regularly.
  9. Follow specific duties outlined by the Director on designated days or projects.
  10. Provide activity reports to the Director of Nursing in their absence.
  11. Assist the Director of Nursing in their responsibilities as instructed.
  12. Conduct monthly surveys on resident care status and prepare reports.
  13. Handle nursing issues when the Director of Nursing is unavailable.
  14. Follow up on recommendations from consulting pharmacists.
  15. Fill in on the floor as needed during staffing shortages or facility issues.
  16. Recognize staff achievements and address performance issues; participate in annual evaluations.
  17. Perform staff appraisals, issue warnings for performance deficiencies, and assess educational needs.
  18. Schedule and deliver in-service training to meet staff development needs.
  19. Manage Utilization Review activities, including assessing residents' levels of care according to Medicare and Medicaid guidelines.
  20. Review certifications for new admissions and monitor ongoing care requirements.
  21. Ensure proper documentation of care to maintain skilled level of care.
  22. Update resident care levels daily as needed.
