Job summary

Empath Health's Tidewell Hospice in southwest Florida is seeking an experienced RN to join our after-hours team, visiting patients in homes and skilled facilities across Sarasota and Manatee counties. Under a licensed physician’s direction, you will assess patients, relieve pain, manage symptoms, document care in the EMR, and support families through bereavement after patient deaths.

Qualifications

• Graduate of an approved nursing program with Florida RN license.
• Current CPR certification required.
• Minimum of 1 year experience in an acute-care hospital environment; hospice RN preferred.
• CHPN encouraged within 3 years; BSN/MSN preferred.

Responsibilities

• Visit patients after-hours to provide clinical, physical, educational and emotional support.
• Under physician direction, assess patients, manage discomfort and provide symptom relief.
• Utilize nursing interventions to maximize patient comfort and quality of life for the patient/family unit.
• Attend patient deaths and provide bereavement support to the caregiver/family.
• Document patient services accurately in EMR to reflect the plan of care and outcomes.
• Review and update the Plan of Care, physician orders, MAR/Pharmacy profile and other records.

Schedule: Sat and Sunday and one evening shift. Coverage Area: Coverage in Sarasota and Manatee counties; Charlotte and DeSoto service counties as needed

What You'll Do

• Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the Triage team (in in the patient home, long term care facility, assisted living facility, hospice house or hospital)
• Under the direction of a licensed physician, performs patient assessment, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.
• Attends patient deaths, provides bereavement support to the patient caregiver/ family, as indicated and performs post mortem care.
• Provides accurate and timely documentation of patient services within electronic medical record to reflect the individualized plan of care and outcomes of interventions.
• Reviews and updates the Plan of Care, physician orders, MAR/Pharmacy profile and other records to ensure patient/family needs are met.

What You'll Need

• Graduate of approved nursing program and current unrestricted Florida RN license.
• Valid and current CPR certification required.
• Minimum one year experience in an acute-care Hospital environment providing direct patient care; previous experience as a Hospice RN preferred.
• CHPN certification is strongly encouraged within three years of employment.
• BSN or MSN preferred.
• An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
