Job summary

Qualifications

• Current, unrestricted RN license in any US state or territory.
• Experience coordinating complex care or case management for Veterans.
• Ability to work with an interdisciplinary team and demonstrate professional communication.

Responsibilities

• Execute streamlined triage and direct timely, coordinated care for Traveling Veterans.
• Serve as the primary point of contact for Veterans and families or caregivers.
• Liaise between Veterans and VA facilities to manage care throughout the process.
• Coordinate care for traveling Veterans, including education, self-management, and customer satisfaction.
• Collaborate with the interdisciplinary team and uphold customer service principles.
