• Execute triage and direct timely care coordination for Veterans.
• Demonstrate proficient nursing practice, decision-making, planning and critical thinking.
• Know Community Care referral process, utilization review standards, and VA guidelines.
• Assess, organize, facilitate, and guide Veterans through internal/direct care and community care options.
• Serve as liaison to internal and community providers to manage Veterans' care during consults.
• Support timely access to care and ensure eligible Veterans receive community care services.
• Coordinate care processes per IVC Field Guidebook, develop care plans, review docs.
