Qualifications

Must hold an unrestricted registered nurse license in the United States.
Experience coordinating complex patient referrals and transitions of care.
Knowledge of VA referral processes and community care options.

Responsibilities

Coordinates care for Veteran referrals through clinical review and triage, ensuring options align with clinical guidelines.
Collaborates with community and VA facilities to facilitate smooth transitions of care and reduce barriers.
Reviews pre-consult testing, labs, imaging and records to support timely referrals and continuity of care.
Supports providers with daily coordination and reports on consult trends and system issues using the nursing process.
