Job summary

U.S. Department of Veterans Affairs is seeking a Clinical Nurse Coordinator to provide clinical care coordination for Veteran referrals within the Office of Community Care.

The role focuses on determining disposition of veteran medical care through clinical review and interdisciplinary collaboration. The position requires a nursing license, appropriate education, and experience in care coordination and case management.

Qualifications

• English language proficiency required for direct patient care per policy.
• Graduate nursing degree from ACEN/CCNE accredited program.
• Bridge program pathway with state licensure before permanent appointment.
• Licensure requirements: full, active, unrestricted nursing license in US territory or DC.
• Foreign nursing education requires equivalency validation.

Responsibilities

• Coordinate care for veterans through referrals and ensure timely pre-consult testing, labs, imaging, and records.
• Provide triage and care coordination for complex patients with veterans and VA/community providers.
• Collaborate with VA facilities and community partners to remove barriers to care.
• Support providers by coordinating dual care across community and VA facilities.
• Assist in developing local referral coordination teams at each VISN facility.
