Job summary

Endeavor Health is seeking a Transitional Care Navigator (RN) to manage case, care coordination and utilization across multiple settings. You will promote understanding of diagnoses, treatment options, and connect patients with resources, ensuring safe transitions and quality outcomes.

This role collaborates with the healthcare team to document care plans, lead utilization management, and support high-risk patients. It is a hybrid, 40-hour-per-week position with 2 onsite days and 3 remote days.

Qualifications

• RN licensed in Illinois with IDFPR requirements.
• 3+ years in utilization review, discharge planning or case management preferred.
• Experience with home/ambulatory services for high-risk patients is beneficial.
• Proficient in EMR systems and data entry, strong MS Office skills.
• InterQual/MCG knowledge and clinical certifications are a plus.
• Excellent communication and collaboration with diverse teams.

Responsibilities

• Guide high-risk patients through diagnosis, treatment and follow-up care.
• Create individualized care plans aligning with patient goals.
• Collaborate with care teams to ensure timely decisions and discharge planning.
• Coordinate across departments to ensure safe transitions and optimize cost of care.
• Provide education on medications, resources, and care options to patients and families.
• Arrange consultations and support services within established protocols.
• Participate in utilization management and monitor LOS and resource use.
