Job summary

ProMedica, a mission-driven health care organization headquartered in Ohio, seeks an RN Rehab Transition Care Manager to oversee patient transitions at Flower Rehab Center. You will engage with physicians, nurses, therapists, leadership, patients, caregivers and the community to ensure smooth admissions and discharges.

The role requires extensive EMR review and active participation in team education regarding admission requirements and processes, with a focus on coordination across care teams

Qualifications

• Registered Nurse from an accredited school of nursing or a bachelor’s prepared allied health professional.
• Strong communication and persuasion skills with patients and healthcare professionals.
• Ability to input, retrieve and analyze EMR data; mid-level Word/Excel skills.

Responsibilities

• Manage all aspects of patient transitions to and from Flower Rehab Center.
• Collaborate with physicians, nurses, therapists, leadership, patients, caregivers and the community.
• Extensive medical record review and active participation in admission-related education and team meetings.
