Job summary

ProMedica is seeking an RN Program Coordinator to oversee patient care coordination from hospital discharge through follow-ups. You will coordinate the plan of care, educate patients on procedures and tests, and review Health Risk Assessments with clinicians.

You will monitor charts for results, discuss them with providers and patients, and manage preoperative clearances as needed. Triage calls and offer health guidance when appropriate.

Qualifications

• Current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification.
• Bachelor of Science in Nursing preferred.
• EPIC experience 2+ years relatable experience.

Responsibilities

• Coordinate the patient plan of care from discharge through all follow-ups.
• Provide education and instructions for procedures and testing ordered by providers.
• Review the patient’s Health Risk Assessment.
• Monitor patients’ charts for test results and review with provider and patient.
• Review and manage preoperative surgical clearance requests with providers.
• Triage patient calls and provide health advice and referrals as appropriate.
