Job summary

ProMedica, a mission-driven health care organization headquartered in Ohio, seeks an RN Rehab Transition Care Manager to oversee patient transitions at Flower Rehab Center. You will engage with physicians, nurses, therapists, leadership, patients, caregivers and the community to ensure smooth admissions and discharges.

The role requires extensive EMR review and active participation in team education regarding admission requirements and processes, with a focus on coordination across care teams

Qualifications

• Registered Nurse from an accredited school of nursing or a bachelor’s prepared allied health professional.
• Strong communication and persuasion skills with patients and healthcare professionals.
• Ability to input, retrieve and analyze EMR data; mid-level Word/Excel skills.

Job Summary:
As the RN, Rehab Transition Care Manager, you manage all aspects of patient transitions to and from Flower Rehab Center. This role will work closely with physicians, nurses, therapists, leadership, patients, caregivers and the community.
This role involves extensive medical record review and you will actively participate in team meetings and education regarding admission requirements and processes.

REQUIREMENTS

• Education : Registered Nurse from an accredited school of nursing or a bachelor's prepared allied health professional.
• Skills : Strong verbal and non-verbal communication and persuasion skills both at patient and healthcare professional levels. Ability to solve interdisciplinary problems in a team environment. Ability to input, retrieve and analyze relevant EMR data. Mid-level Microsoft Word and Excel skills.
• Years of Experience : Minimum of 2 years clinical practice experience in a patient care setting.
• License : Registered Nurse, Licensed in the state of Ohio or Licensed or Registered Allied Health Professional
• Certification : CPR

PREFERRED REQUIREMENTS

• Education : Bachelors, Masters or Doctorate degree in a nursing or allied health profession.
• Skills : Ability to work with a variety of internal/external customers and healthcare providers effectively and diplomatically.
• Years of Experience : 3-5 years clinical practice experience in a patient care setting.
• License : N/A
• Certification : Certified Rehabilitation Registered Nurse (CRRN), Allied Health Professional certification in IRF related diagnoses and/or patient management
