Job summary

A ProMedica RN Acute Care Navigator oversees patient care, ensuring seamless progression and the appropriate level of care for their caseload. You focus on optimizing length of stay, preventing readmissions and compliance with value‑based programs.

You collaborate with interdisciplinary teams to conduct initial assessments and develop discharge plans, and you provide patient education on medications, treatments, and post‑discharge care.

Qualifications

• Must have either a BSN, or 5 years’ applicable nursing experience.
• A current, active, and unrestricted licensure as a Registered Nurse in the state of practice is required

Responsibilities

• Oversee patient care and ensure seamless care progression for caseload.
• Coordinate and develop discharge plans with interdisciplinary teams.
• Educate patients on medications, treatments, and post‑discharge care; monitor progress and review health records.
• Escalate issues to leadership to ensure smooth transitions and patient outcomes.
