Job summary

Trilogy Health Services, LLC is seeking a Registered Nurse (RN) to provide direct, high-quality clinical care and to serve as a Team Leader for patient care staff. The role emphasizes leading a team, coordinating care, and ensuring medication administration and documentation meet policy standards.

Responsibilities include daily resident assessments, care planning, and timely escalation of any changes in condition, with emphasis on documentation and collaboration with the care team.

Qualifications

• Must have current, valid state RN license.
• Current CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure correct administration of medications and treatments, documentation, family teaching, care planning and patient care conferences.
• Use the nursing process to deliver care and ensure continuity from admission to discharge and return to family and community.
• Visit residents daily to observe status and notify charge nurse/physician when a resident's condition changes.
• Identify resident problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting.
