Qualifications

Responsibilities

• Lead a team of direct care providers to ensure medications, treatments, and care planning.
• Ensure continuity of care from admission through discharge and return to family and community.
• Visit residents daily to observe status and notify supervisor of changes.
• Identify resident problems and initiate lifesaving measures in emergencies if needed.
• Perform administrative duties such as forms, reports, evaluations, and charting.
