Trilogy Health Services, LLC is seeking an experienced Registered Nurse (RN) at our Silver Oaks Health Campus in Columbus, Indiana. The RN will lead a direct care team, provide high-quality clinical care, and ensure accurate documentation and care planning in collaboration with the nursing leadership.

The role emphasizes patient-centered care, adherence to policies, and timely communication with physicians and family members.

Formación

• Must hold a valid state RN license.
• Current CPR certification required.

Responsabilidades

• Lead a team of direct care providers to ensure medications, treatments, documentation, family teaching, care planning and patient care conferences are performed per policy.
• Use the nursing process to deliver care and ensure continuity from admission through discharge and return to family/community.
• Visit residents on assigned unit daily to observe status and notify charge nurse/physician/Director of Nursing if conditions change.
• Identify resident problems and emergencies and initiate immediate lifesaving measures when physician is unavailable.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting.
