Job summary

Trilogy Health Services in Lancaster, Ohio, is seeking a Registered Nurse (RN) to lead a patient care team and deliver direct, high-quality clinical care. The RN will supervise direct care staff, manage care planning, and ensure documentation and care conferences meet policy standards.

Responsibilities include applying the nursing process from admission through discharge, daily resident visits, and recognizing emergencies for timely intervention.

Qualifications

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications and treatments are executed correctly, with documentation, family teaching, care planning, and patient care conferences in line with policies.
• Utilize the nursing process to deliver care and ensure continuity from admission to discharge and return to family/community.
• Visit residents daily on assigned units to observe and evaluate status and notify charge nurse/physician/DON about changes in condition.
• Identify resident problems and emergencies and initiate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting as needed.
