Cobble Hill Health Center, a not-for-profit facility in New York, is seeking an experienced Registered Nurse (RN) to join our team. The RN provides expert care in activities of daily living, supports residents’ independence, and mentors care staff.

Responsibilities include leading nursing care from admission to discharge, supervising CNAs and LPNs, coordinating with the care team to create individualized plans, and ensuring timely responses to changes in residents’ conditions in line with

Qualifications

• Current RN NY License.
• CPR Certification.
• Nursing Degree (ASN or BSN).
• Experience in skilled nursing facility or long-term care preferred.

Responsabilités

• Serve as the nursing leader and educator on the unit. Directs all care related items from admission through discharge.
• Supervises CNAs and LPNs.
• Collaborate with the interdisciplinary care team to create an individualized plan of care.
• Respond to changes in condition to prevent re-hospitalizations and emergencies.
• Educate and reinforce nursing competencies in wound care, ostomy care, catheter care, medication administration, and related procedures.
• Perform tasks such as IV insertion, PICC line care, GT and suprapubic catheter reinsertion, initiate care plans and review data.
